5. Age proof document in original (Admit Card / Marksheet of MP/HS/ICSE/CBSE).
6. Attested photocopy of age proof document (Admit Card / Marksheet of MP/HS/ICSE/CBSE).
7. Caste certificate in original in case of SC / ST candidates issued by the competent authority in West Bengal only.
8. Physically Challenged certificate in original.
9. Attested photocopy of the Physically Handicapped Certificate.
10. Attested photocopy of the Caste certificate in original in case of SC / ST candidates.
11. Two (2) copies of stamp-sized photograph.
12. College copy of the Challan stamped by the Bank on receipt of money.
